The mission of our
Family
Case Management
program is to help
pregnant women who may be at risk to have healthy children with
healthy futures. Our services target pregnant women and infants
up to one year of age, or medically high-risk children up to 5 years
of age. Healthy Moms/Healthy Kids Case Managers work to connect
pregnant women to early prenatal care, and link infants and children
to the important health care they need. Spanish translators are
available upon request.

HM/HK also oversees
Health Works Illinois, a program that ensures wards of the state
receive proper healthcare. HM/HK hosts the Targeted Intensive Prenatal
Case Management program. Because of our high rate of risk associated
with infants born here, Vermilion County is one of 11 sites selected
in the state to focus on high risk assessment of pregnant women,
and to intensify case management of those who qualify. Additionally,
HM/HK provides assistance with the Teen Parent Services program
which provides comprehensive services to pregnant or parenting teens
under the age of 21 so they have healthier children and complete

Familia National Hispanic Family Health Helpline
A
Bi-lingual telephone Health Help Line is now available for Hispanic
families. This toll free service from the Department of Health
and Human Services will connect Hispanic families to the community
health services they may need, with information provided in both
Spanish and English. Call
