ermilion County Health Department is the Local Registrar for births and deaths in our county. We are authorized to provide certified copies of birth and death certificates, which are recorded and filed here at the Health Department, 200 South College Street, Suite A, Danville, IL 61832 (217-431-2662) .

Our records cover births and deaths from
June 1, 1983
and beyond . Certified copies of birth and death certificates before June 1, 1983 are available from the County Clerk's office at the Vermilion County Courthouse Annex in Danville
(217-431-2616).

Before your request for a certified copy can be considered, you must specify your eligibility to obtain it. Illinois law state that copies of death records may be issued only as listed below:
Upon the specific written request for a certified copy by a person, or his duly authorized agent, having a personal or property right interest in the record.
All information marked "REQUIRED" on the application must be completed in order for us to process your request. If you believe you are eligible, please complete the application and mail it with a check for the correct amount to:

The current fee for a birth record search and first or initial certified copy is $12.00; an additional copy is $2.00. Death certificates are $14.00; additional copies are $4.00, which includes the state-levied surcharge.
