An
aging population encourages us here at the Vermilion County Health
Department to continue exploring ways in which we can meet the needs
of our senior citizens. Our Community Care program is one way to
help older adults remain in their homes for as long as possible.
Based on the 2000 census, we know that of Vermilion County's 83,919
residents, 13,425 or 16% are age 65 and older. There are 9,506 households
(28.5%) in the county with individuals who are age 65 and older.
Our
Community Care home-based program has been providing services since
1983.
The
Vermilion County Health Department's Community Care program is for
residents 60 years of age or older; who are unable to continue handling
many of the acts of daily living; the program is income based.
Following
a determination of eligibility, our Community Care case managers
partner with local agencies to create a support system which allows
the individual to remain at home and independent for as long as
possible. The Community Care program also provides pre-screening
for long term care. By mandate, the state of Illinois requires an
assessment prior to an individual's admittance to a nursing home,
regardless of income. The initiative is designed to control the
growth of state spending for Medicaid, and to inform older adults
and their caregivers about their options, and the choices available
for long term care. During 2004, CCU completed 664 nursing home
pre-screenings.
