Senior Services:

Case Coordination Care Unit for Title XX Community Care Services

Vermilion County (IL) Health Department


The Vermilion County Health Department was designated by the East Central Illinois Area Agency on Aging in 1983 as the Case Coordination Unit for Community Care clients and service vendors in Vermilion County. OUr responsibilities focus upon assisting qualifying seniors citizens to stay in their homes and continue independent lives for as long as possible. Our staff is also charged with screening anyone entering a licensed nursing home in Vermilion County. In 2008, case managers completed nearly 800 mandated nursing home prescreenings

The Vermilion County Health Department's Community Care program is for residents 60 years of age or older; who are unable to continue handling many of the acts of daily living; the program is income based.

Following a determination of eligibility, Community Care case managers partner with local agencies to create a support system which allows the individual to remain at home and independent for as long as possible.
